| Additional Requirements: | | - Alarm installation activity: A copy of the current, active C-12 contractor's license and /or an L67 low voltage communications license, issued by the state registrar of contractors.
- Alarm monitoring activity: Proof of Underwriter's Laboratories (UL) or Factory Mutual (FM) listing.
- Certificate of liability insurance, evidencing errors and omissions insurance and combined general comprehensive insurance in the minimum amount of $1 million, and must specifically cover alarms systems. This insurance coverage shall remain in full force and effect through out the term of the license.
- Proof of current Primary Alarm Business license with participating reciprocal city (Chandler, Mesa, Phoenix, or Tucson).
- Proof of age(Ex. Driver's license, Birth certificate, etc.)
- Proof of eligibility to work in the United States. (Ex. Driver's License, Birth certificate, documentation from U.S. Dept of Justice, Immigration & Naturalization Service, etc)
