To use this service, you need your billing statement showing the total amount due. Only those statements listed in the "Form Name" list below can be submitted through this service.
- Changes to your account information are not accepted through this service. Please follow the directions on the statement.
- Payments for Privilege Tax Liability are not accepted through this service. The forms not accepted are titled "Transaction Privilege Use Tax Return" and "Privilege (Sales) & Use Tax Statement".

Form Name:
Enter the name of the Statement located at the top of the document.
License/Permit#:
Enter the License or Permit number printed on the document just below the License Number or Permit Number heading.
Confirm License/Permit#:
Re-Enter the License or Permit number printed on the document just below the License Number or Permit Number heading.
Business Address:
Enter the first line of the Business address, it is located under the business name on the left of the statement.
